术中评估
以膀胱镜筛查尿路损伤 — 现已提出对接受妇科大手术的女性术中常规行膀胱镜检查,以作为尿路损伤的二级预防措施[2,3]。膀胱镜检查所发现的损伤多于尿路肉眼观察。肉眼观察发现的膀胱损伤多于输尿管损伤。2项常规使用了膀胱镜的前瞻性子宫切除术(经腹部、阴式或腹腔镜)病例系列研究(n=839和471例)说明了这一点[2,4]。尿路损伤的肉眼观察未能发现87%-93%(7/8和14/15)的输尿管损伤和62%-65%的膀胱损伤(15/24和11/17)—这些随后通过膀胱镜检查得以诊断[2,3]。每项病例系列研究中仅有1例损伤未被肉眼观察和膀胱镜检查发现,这2例患者术后都出现了泌尿生殖道瘘。

术中常规使用膀胱镜检时,术后发现的尿路损伤发生率似乎减少。一项针对47项妇科手术回顾性研究的系统评价评估了术中常规使用膀胱镜检对尿路损伤诊断的影响[1]。术中常规使用膀胱镜检的研究中术后诊断的尿路损伤发生率低于术中未常规使用膀胱镜的研究:膀胱损伤为0.8例/1000例手术 vs 2.2例/1000例手术,输尿管损伤为1.0例/1000例手术 vs 2.2例/1000例手术。该发生率系几种不同手术(如,子宫切除术、脱垂修补术)的数据联合得出,可能掩盖了手术特异性差异。该分析也可能受限于其使用的回顾性数据—可能遗漏了并发症(例如,去其他机构就诊的患者)。
