引言

未行包皮环切或已行部分包皮环切的男性在将包皮牵拉至阴茎头后，发生静脉和淋巴回流障碍，且包皮不能恢复至正常位置时，则发生嵌顿性包茎(图片 1)。这是一种泌尿科急症，可影响阴茎头的血供，甚至可能导致永久性损伤和坏疽。

对于婴幼儿，嵌顿性包茎通常是儿童自身操作或看护人在尝试清洁时因为未受到正确指导而不适当地牵拉包皮所致。对于性活跃的青少年和成年男性，性交是潜在的促发因素。医护人员在膀胱镜或膀胱导尿术后未将包皮复位至阴茎头，可发生医源性嵌顿性包茎。

嵌顿性包茎复位一般通过疼痛控制、用局部措施减轻水肿，以及手法或微创操作完成。少数微创复位失败或长时间嵌顿性包茎并发阴茎头坏死或感染的患者，需要泌尿科紧急会诊。
