急性ST段抬高型心肌梗死(ST elevation myocardial infarction, STEMI)患者处理的第一步是迅速识别,因为在就诊后很快进行再灌注治疗的获益最大。对急诊科就诊的、有胸痛且怀疑为急性冠脉综合征(acute coronary syndrome, ACS)的患者,STEMI的诊断可采用心电图(electrocardiogram, ECG)确证。生物标志物在早期可能正常。 (参见“急性心肌梗死的诊断标准”和“急诊科疑似急性冠状动脉综合征的初始评估与治疗”)
一旦诊断急性STEMI,患者的早期处理包括同时达到几个目标:
●缓解缺血性疼痛
●评估血流动力学状态并纠正存在的异常
●采用直接经皮冠状动脉介入术(percutaneous coronary intervention, PCI)或纤维蛋白溶解开始再灌注治疗
